Shocking Evidence of Mutilation, Evidence Concealment, and Gross Violations of Forensic Medical Principles

 As medical specialists, we have conducted a structured review of publicly available videos and photographs documenting injured and deceased individuals during recent events in Iran. Our assessment is based on established medical principles, emergency medical standards, principles of forensic pathology and shared ethical obligations of healthcare professionals worldwide.

The observations outlined below demonstrate patterns that have been independently identified and discussed by physicians, nurses, emergency medical personnel, forensic pathologists, mortuary technicians, autopsy assistants, post-mortem and biopsy workers, as well as personnel working in hospitals, morgues and forensic medicine centers.

1. Evidence of hospital medical intervention before death: In several cases of deceased individuals, medical tools and equipment have been observed that are used exclusively in clinical care and treatment environments, including “peripheral intravenous catheters (IV cannulas)”, “indwelling urinary catheters (Foley catheter)”, “nasogastric tubes (NG)”, “oxygen delivery tubes” and “electrodes and ECG/EKG monitoring leads”.

The presence of these devices confirms that these individuals were alive at the time of receiving care or medical evaluation and were undergoing treatment or monitoring in hospital or emergency settings before death.

From a medical perspective, these findings demonstrate that death occurred after medical intervention began. The continued presence of these devices on bodies outside hospital environments raises deep and serious concerns that injured individuals, or those still alive, may have been removed from hospitals or killed after receiving medical care; a situation that constitutes “serious violation of the principle of medical neutrality and patient protection.”

2. Lack of death certification before body transport and transfer: Examination of multiple videos and images revealed the presence of biological signs inconsistent with confirmed death, including “observation of limb movement inside body bags”, “skin coloration consistent with continued blood circulation”, “absence of some expected post-mortem changes in certain individuals.”

From medical and legal perspectives, death certification requires formal assessment of vital signs, including “absence of pulse”, “breathing and cardiac activity” followed by formal announcement of death by qualified medical personnel.

The evidence reviewed strongly suggests that standard procedures for confirming death before placing individuals in body bags or transferring them for burial or storage in morgues were not followed. This report indicates initial violation of principles of medical ethics, legal standards and human rights.

3. Abandonment of bullet wound victims without care: Multiple cases have been documented in which individuals with multiple gunshot wounds were abandoned among deceased bodies without any treatment, and no evidence of triage or provision of emergency medical care is observed.

From an emergency medical perspective, gunshot wounds in many cases are survivable if bleeding is controlled promptly and medical intervention is provided, and patient survival depends on rapid assessment and immediate initiation of treatment.

The observed abandonment of injured individuals indicates deliberate failure to provide life-saving care, resulting in preventable deaths. This situation constitutes abandonment of medical care and represents a clear violation of internationally accepted standards in trauma care, disaster response and humanitarian medicine.

4. Extensive abdominal incisions inconsistent with standard care: In multiple cases, bodies with extensive longitudinal incisions along the midline of the abdomen extending from the lower chest region to the lower abdomen have been observed. Such incisions are typically associated with: “complete forensic medical autopsy” or “major abdominal surgeries.” However, in the cases reviewed, these incisions were observed outside documented forensic medical protocols and were often accompanied by other signs of improper body handling and displacement.

From a forensic medical perspective, two recognized and serious concerns are raised:

  1. Illegal organ removal: Certain organs can only be used for transplant or sale if removed from the body while circulation is still maintained or immediately after cardiac arrest.
  2. Intentional destruction of forensic medical evidence: Extensive damage to abdominal structures can conceal internal signs of trauma, bleeding and injuries from severe impact, thereby preventing future forensic analysis and concealing evidence of severe physical abuse.

5. Evidence of severe physical assault prior to death: In multiple cases, bodies show findings consistent with severe and extreme physical violence. The combination of extensive body destruction, remnants of hospital equipment on bodies and absence of proper forensic investigation raises serious concern that remains have been deliberately manipulated to conceal evidence of severe beating and torture.

Such actions disrupt medical accountability and violate fundamental principles of forensic medicine and professional ethics.

6. Decapitation and severe mutilation: In some cases, complete separation of the head from the body has been observed. From a medical and forensic medical perspective, decapitation has no connection to medical care or forensic medical procedures. This action represents severe and deliberate violence and prevents identity verification and forensic reconstruction of events. These findings suggest that actions taken went beyond any legitimate law enforcement or crowd control activities.

7. Improper displacement and mass placement of bodies: In several documented cases, multiple bodies have been placed inside a single body bag. This action violates international morgue and forensic medical standards, makes individual body identification impossible, causes cross-contamination of forensic medical evidence and disrupts the process of determining cause and manner of death.

This manner of treatment demonstrates systematic disregard for human dignity, medical protocols and legal procedures.

8. Injuries consistent with chemical substance contact: In addition to traumatic injuries, findings have been observed in multiple individuals that are medically consistent with exposure to toxic chemical substances, including: “skin lesions resembling chemical burns without thermal injury”, “unusual skin discoloration”, “severe irritation of eyes and airways” and “respiratory distress”.

These patterns differ from common injuries caused by bullets or direct impact and are consistent with contact with toxic chemical agents, as confirmed by emergency physicians, pulmonologists, dermatologists and forensic pathologists.

9. Severe thermal destruction of remains: In some cases, remains showed extensive and severe burn injuries resulting in destruction of soft tissues and exposure of skeletal structures. In such circumstances, conducting any valid medical or forensic medical examination is severely limited or practically impossible, and as a result, crucial evidence for determining cause and time of death is effectively destroyed. This type of treatment and handling demonstrates systematic disregard for human dignity, medical protocols and legal procedures.

10. Professional medical interpretation: In summary, the compiled findings demonstrate the following:

1. Violation of the principle of medical neutrality

2. Transfer or killing of patients after initiation of medical treatment

3. Lack of death certification before body displacement and handling

4. Deliberate abandonment of injured individuals

5. Severe physical violence and mutilation

6. Suspected illegal organ removal

7. Destruction and concealment of forensic medical evidence

8. Mass and improper displacement and handling of bodies
